Position Overview:
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team in Tunkhannock. In this role, you will provide comprehensive care to patients in their homes, ensuring the highest standards of nursing practice are met. You will be responsible for assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records.
Key Responsibilities:
- Conduct thorough assessments of patients' health conditions in their home environments.
- Develop personalized care plans and collaborate with multidisciplinary teams to deliver optimal patient care.
- Administer medications and treatments as prescribed by the physician.
- Provide education and support to patients and their families on disease prevention and health maintenance.
- Monitor patient progress and modify care plans as necessary to achieve health goals.
- Maintain accurate, detailed reports and records in accordance with regulatory requirements.
- Ensure compliance with all healthcare regulations and quality standards.
Qualifications:
- Current Registered Nurse (RN) license in the state of Pennsylvania.
- 1+ years of acute care experience.
- Know OASIS from experience in-home health visits
- Require Home Health Experience.
- Strong assessment and clinical skills with a focus on patient-centered care.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
- Ability to work independently and manage time effectively in a home health environment.
